Birth Certificate Title

A. WHO IS ELIGIBLE TO OBTAIN A BIRTH CERTIFICATE COPY?

  • Applicant must be born in one of the 5 boroughs of New York
  • The person named on the birth certificate.
  • A parent of the person named on the birth certificate (requesting parent's name must be on birth certificate).
  • Person with Official Agency Letter on the agency's letterhead (eg. Police Department or Social Services).

B. COMPLETE BIRTH CERTIFICATE APPLICATION

The Birth Certificate Application should be completed and signed.

C. LETTER OF AUTHORIZATION

This is the release letter authorizing It's Easy, Inc. to submit the paperwork and collect the Birth Certificate copy on your behalf. This Authorization Letter (PDF) must be printed, notarized and signed by applicant.


C. IDENTIFICATION REQUIREMENTS

The Birth Certificate Application must be submitted with copies of either 1, 2 or 3:

  1. Valid photo-ID:
    • Driver license
    • Non-Driver Photo-ID Card
    • Vaild Passport
  2. Inmate photo-ID with Release Papers
  3. Two of the following showing applicant's name and address:
    • Utility/Telephone bills
    • Letter from a Government Agency dated within the last six (6) month


D. SERVICE & RETURN SHIPPING FEES

  • The fee is $15.00 per birth certificate copy by check or money order payable to N.Y.C. Department of Health and Mental Hygiene.
  • It's Easy, Inc. service fee of $75.00 for *SAME DAY SERVICE or $35.00 for *NEXT DAY SERVICE can be paid by money order or personal checks (no starter checks).
  • The Shipping fees are $25.00 for Overnight Delivery or $35.00 for Saturday Delivery
  • You may deliver the documents in person.


E. SHIP DOCUMENTS to IT'S EASY, INC.

Ship the following via Fedex, UPS, or DHL only:

  1. Identification Requirements
  2. Birth Certificate Application
  3. Letter of Authorization
  4. Payment to "N.Y.C. Department of Health and Mental Hygiene" ($15.00)
  5. Payment to "It's Easy, Inc." ($75.00 - Same Day; $35.00 - Next Day Service)

Please ship your package via Fedex Priority Overnight, UPS Next Day, DHL Next Day 10:30 am or DHL Same Day service only. Do not use U.S. Postal Service Express Mail to ship the items listed above.
