Death Certificate Title

A. WHO IS ELIGIBLE TO OBTAIN A DEATH CERTIFICATE COPY?/

  • The spouse, parent or child of the deceased
  • Other persons who have a documented lawful right or claim; documented medical need; New York City Court Order

B. WHAT IS A LAWFUL RIGHT OR CLAIM?

If the applicant is not the spouse, parent or child of the deceased, a lawful right or claim must be documented. A death record needed by the applicant to claim a benefit, would be an example of a lawful right or claim. Documentation would consist of an official letter from the agency verifying that to process the claim they require from the applicant a copy of the requested death record.

C. COMPLETE DEATH CERTIFICATE APPLICATION

The Death Certificate Application must be completed and signed.

D. IDENTIFICATION REQUIREMENTS

The Death Certificate Application must be submitted with copies of either from 1 or 2:

  1. Valid photo-ID:
    • Driver license
    • Non-Driver Photo-ID Card
    • Passport
    • Employment ID
  2. Two of the following showing the applicant's name and address:
    • Utility or telephone bills
    • Letter from a government agency dated within the last six (6) months

E. SERVICE & RETURN SHIPPING FEES

  • The fee is $15.00 per death certificate copy by check or money order payable to N.Y.C. Department of Health and Mental Hygiene.
  • It's Easy, Inc. service fee of $45.00 can be paid by money order or personal checks (no starter checks).
  • The Shipping fees are $25.00 for Overnight Delivery or $35.00 for Saturday Delivery
  • You may also pick-up your document(s) in person.


F. SHIP DOCUMENTS to IT'S EASY, INC.

Ship the following via Fedex, UPS, or DHL only:

  1. Identification Requirements
  2. Death Certificate Application
  3. Payment to "N.Y.C. Department of Health and Mental Hygiene" ($15.00 per copy)
  4. Payment to "It's Easy, Inc." ($75.00 - Same Day; $35.00 - Next Day Service)
  5. You may deliver the documents in person.

Please ship your package via Fedex Priority Overnight, UPS Next Day, DHL Next Day 10:30 am or DHL Same Day service only. Do not use U.S. Postal Service Express Mail to ship the items listed above.
